What is hair restoration

Three tools, one protocol.

It's a combination protocol designed for the early-to-moderate thinning where you can still see follicles but they're producing finer, weaker hair. Three components work together — Sylfirm X pulsed-wave RF microneedling creates controlled microchannels in the scalp at adjustable depths; autologous platelet-rich plasma adds concentrated growth factors from your own blood; Benev exosomes add serum-borne signaling vesicles that further support follicle activity.

The combination is more effective than any single element alone. Microneedling on its own creates the channel and a modest local response; PRP adds growth-factor signaling; exosomes add additional cellular signals. We use them together because that's what the protocol literature and our results support.

This is not a hair transplant. We don't move follicles. We work with the follicles you have — supporting growth phase, reducing miniaturization, encouraging fuller terminal hairs. For advanced cases where follicles are gone, we'll refer to surgical hair transplant rather than oversell our protocol.

The hair restoration protocol at Reviv Med Spa in Millbrae is performed by our Medical Director Dr. Gayle Misle, MD, and our Aesthetic Nurse Megan Sosnick, RN — both with extensive training in PRP processing and Sylfirm X RF microneedling technique. Sterile single-use kits, closed-system processing, and careful microneedling depth control are standard for every visit.

Your visit is about 60 to 90 minutes — consultation review, blood draw, topical numbing while the centrifuge processes PRP, Sylfirm X across the target areas (hairline, vertex, parietal regions, or wherever miniaturization is occurring), then PRP and exosome application through the channels. The initial series is typically 3 to 6 sessions spaced 4 to 8 weeks apart, with maintenance every 6 to 12 months once goal is reached.

What to expect

From day zero, to month nine.

Day 0

Treatment day. Small blood draw, centrifuge to separate platelet-rich plasma, then scalp injections (or microneedling delivery). Total visit: 60–90 min. Mild scalp soreness for a day.

Weeks 1–4

A brief shedding phase is normal as resting follicles cycle out — this is the protocol working, not failing. Continue scalp care, gentle handling, and any prescribed topicals.

Months 3–4

Early regrowth begins — finer baby hairs along the hairline and through thinning zones. Reduced shedding becomes noticeable. The follicles are responding to the growth-factor signal.

Months 6–9

Visible density returns. Patients typically complete a 3- to 6-session initial series and then refresh every 6 to 12 months to sustain the result. This is a slow-build protocol — plan months, not weeks.

A good fit if
  • Early to moderate androgenetic alopecia (male or female pattern thinning)
  • Postpartum shedding (telogen effluvium) and recovery from medication-related shedding
  • General density loss with age — not yet bald spots, but visibly thinner
  • Patients combining the protocol with consistent scalp care, nutrition, and stress management
Not for you if
  • Advanced bald spots (Norwood 6+ in men; Ludwig III in women) — surgical hair transplant is a better option
  • Active alopecia areata or other autoimmune scalp conditions
  • Scarring alopecias, active scalp infection, or recent isotretinoin within 6 months
  • Pregnant or breastfeeding patients, and those with bleeding disorders

How long does it take to see results?

Hair restoration is gradual. Most patients notice reduced shedding around weeks 8 to 12. Visible new growth (baby hairs filling in along the hairline or at the crown) typically becomes apparent at months 3 to 6. Hair restoration treatments work with the natural growth cycle — there is no instant result.

How much does the program cost?

Does the treatment hurt?

Topical numbing applied for 30 minutes brings most patients to a 3 to 4 out of 10 during microneedling. The blood draw is the same brief sensation as a routine blood test. The application of PRP and exosome serum after microneedling is painless. Mild scalp tenderness for 24 hours is common.

Is there downtime?

Minimal. Mild redness and scalp tenderness for 24 to 48 hours. No hair washing or styling for 24 hours. Avoid swimming and direct sun on the scalp for a week. Most patients return to work the same day.

What kind of hair loss does this address?

Best results in early to moderate androgenetic alopecia (male and female pattern hair loss), postpartum shedding, telogen effluvium recovery, and overall thinning. Less effective in advanced cases where follicles have miniaturized completely. We screen at consultation and tell patients honestly when surgical hair transplant or medical management (finasteride, minoxidil) might be a better fit.

Can men and women both get this?

Yes. We treat male and female pattern thinning with the same combination protocol, adjusted for the typical pattern of loss in each. For men, the focus is often the temporal recession and crown; for women, often the part line and overall density.

How does this differ from a hair transplant?

A hair transplant surgically moves follicles from a donor area (typically the back of the scalp) to thinning areas — it adds follicles. Our protocol works on the follicles you have: stimulating activity, supporting growth phase, reducing miniaturization. The two approaches address different problems and can complement each other in advanced cases.

Are there side effects?

Most patients tolerate the protocol well. Possible side effects: scalp redness, mild bruising at the draw site or microneedle channels, transient itching during early growth phase. Rare reactions to the topical anesthetic or the exosome serum. Sterile single-use kits and careful technique keep risk low.

How is this different from over-the-counter minoxidil or oral finasteride?

Different mechanism, different delivery. OTC minoxidil and oral finasteride are pharmacologic — they work systemically (or topically as in minoxidil) to extend growth phase or block DHT. Our protocol delivers growth factors and biological signals directly to follicles via microchannels. We do not prescribe finasteride or minoxidil at Reviv; many patients combine our protocol with one of these under their primary care physician.

Who should not get this protocol?

Patients with active scalp infection (ringworm, folliculitis), bleeding disorders, autoimmune conditions affecting the scalp, certain blood-borne infections, and pregnant or breastfeeding patients. Patients on retinoid medications (isotretinoin) within 6 months. We screen carefully at consultation.
