Sylfirm X is a radiofrequency microneedling device by Viol (Korea), FDA-cleared in the United States. It pairs gold-tip insulated needles with two distinct RF energy modes — Pulsed Wave (PW) and Continuous Wave (CW) — to address pigment, redness, scars, texture, pores, and mild laxity in one device.
The needles create controlled micro-channels in the skin while delivering RF energy at a precise dermal depth. The micro-channels stimulate the skin's wound-healing response; the RF triggers collagen remodeling underneath. The two effects together produce more change than needles or RF alone.
The big difference vs. light-based devices: Sylfirm X is safe across the full Fitzpatrick range (I–VI). Lasers and IPL carry pigment risk on darker skin tones; RF does not. That's the reason we added Sylfirm X to the practice — to serve patients who can't safely use a laser.
Two modes, one protocol per concern.
At Reviv Med Spa in Millbrae, every Sylfirm X session starts with a face-to-face skin assessment — Fitzpatrick type, pigment history, scar pattern, what we're trying to change. The setting and mode are chosen for what we're treating, not a one-size-fits-all default. Pigment-sensitive cases (melasma, PIH, darker skin tones) get Dr. Misle's direct oversight on settings.
Sylfirm X has two RF modes — we use them differently:
- Pulsed Wave PW mode
- Short, controlled energy bursts. Gentler. Used for melasma, post-inflammatory hyperpigmentation, persistent redness, and rosacea — anywhere pigment safety is the priority.
- Continuous Wave CW mode
- Sustained energy at depth. Stronger remodeling. Used for atrophic acne scars, stretch marks, deeper texture, and lower-face laxity.
- Gold-tip needles insulated
- Insulated along the shaft so the surface (epidermis) stays intact while RF energy lands at the chosen depth in the dermis.
- Adjustable depth 0.3–4.0 mm
- Depth is set per area — shallower for delicate eye-area or thin skin, deeper for scars and laxity.
Topical numbing cream is applied for 30 minutes before we start. The treatment runs 30–60 minutes depending on area size. After treatment we apply a soothing serum and send you home with a written aftercare protocol — gentle cleanser, healing balm, mineral SPF, and the list of actives to avoid for 5–7 days. Most patients return to work the next day with a tinted SPF.
Standardized progress photos at each session let you see the change side-by-side rather than guessing.

Pigment + redness in one device
Most lasers force a choice between pigment and vascular targets. Sylfirm X's Pulsed Wave mode handles both, even on melasma-prone skin.
Acne and traumatic scars
Atrophic acne scars, rolling scars, and traumatic surgical scars all respond to dermal RF energy and microneedling — usually a series of 3–4 over several months.
Mild laxity without surgery

How is Sylfirm X different from regular microneedling?
Traditional microneedling uses needles alone — small punctures stimulate a wound-healing response that builds collagen on the surface. Sylfirm X adds radiofrequency energy delivered through gold-tip insulated needles, so the heat is placed at a precise depth in the dermis without injuring the surface. The added RF makes it more effective for laxity, scars, and pigment than needles alone.
What does "PW vs. CW" mean?
Sylfirm X has two energy modes. Pulsed Wave (PW) delivers very short, controlled bursts — gentler, safer for pigment, and the protocol of choice for melasma, post-inflammatory hyperpigmentation, and persistent redness. Continuous Wave (CW) delivers sustained energy at depth — better for scar remodeling, deeper laxity, and texture. Most patients use a blend across the series, customized to their goals.
Is Sylfirm X safe for darker skin tones?
Yes. Because Sylfirm relies on radiofrequency rather than light, it's safe across all Fitzpatrick skin types (I–VI). Light-based devices (lasers, IPL) carry pigment risk on darker skin; RF microneedling does not. This is one of the most important reasons we added Sylfirm X to the practice.
Does it hurt?
Most patients describe it as a strong vibration with mild pinpricks. We apply a topical numbing cream for 30 minutes before treatment, which makes most of the session comfortable — a 3–5 out of 10. Sensitive areas like the upper lip can feel sharper.
How many sessions do I need?
Most concerns improve meaningfully after a series of 3–4 sessions spaced about 4 weeks apart. Acne scars and stretch marks sometimes need a longer series. We map the plan at consultation and reassess at each session.
How soon will I see results?
You'll see initial smoothing and tone improvement within 2–4 weeks. Collagen remodeling continues for 3–6 months — the full result shows after the complete series finishes. We track standardized photos at each follow-up so you can see the change.
Is there downtime?
Expect 24–48 hours of pink-to-red skin and a sandpaper texture for 2–3 days. Most patients return to work the next day with a tinted mineral SPF. No actives (retinol, AHAs, vitamin C) for 5–7 days. We send a written aftercare protocol home with you.
How does it compare to Halo or MOXI?
Halo and MOXI are hybrid fractional lasers — they remove a tiny amount of surface tissue along with their dermal effect. Sylfirm X is non-ablative; it leaves the surface intact. Sylfirm is generally safer for darker skin and pigment-prone patients; Halo can deliver a more pronounced surface texture change in patients whose skin tolerates it. We use both at Reviv and will recommend the right one for your skin type and goals.

Can I combine Sylfirm X with other treatments?
Yes — Sylfirm pairs well with Botox (placement spaced apart), filler (placed on a different visit), and Skinvive (placed 2–4 weeks apart). We'll plan the combination at consultation. Some lasers and Sylfirm should be spaced apart to let the skin recover between treatments.
